The official Navy access form below must be completed for each guest 18 years or older who do not have DoD issued identification. Each form must be processed by Naval Station Norfolk in order for us to issue tickets to the ceremony and grant access to the base. The form must be printed, completed in its entirety (highlighted fields), signed and dated, and returned to us (scan or photograph) immediately if possible but by no later than Monday, April 6th. Please note that the form has both a front and back and your social security number is required in block 14. Naval Station Norfolk will not accept forms that are digitally signed or contain missing information. To safeguard your personal information, it is imperative that you use one of the means below to submit your form(s).
DoD Safe: DoD Safe is the Department of War’s secure platform to encrypt and send files and is the safest and most expeditious way to ensure that we receive your completed forms and safeguard your personal information. Each of you will receive an email shortly from NoReplyTo@mail.mil requesting you to upload your form using a unique request code provided in the email. Please carefully follow the instructions provided below on this site. Ensure that you click the “Encrypt every file” box in the space provided. When prompted to set an encryption passphrase, please use DDG124CHARGEON so that we can retrieve your file once you have uploaded it.
